(Terms and Conditions for Testing)

一．承检方根据委托方要求对委托方提供的样品进行检测，出具检测报告。委托方按合同书要求提供可供检测的有效样品，按合同约定的时间、金额支付检测费用。

Tester will subject to the clients’ requirements test any sample offered by them, issue the test report. The client shall supply legal sample and pay the test charge as stipulated in the contract.

二．委托方通过自送、委托及邮寄等方式提供的检测样品必须是完整的、无破坏的、安全及有效的，可用于检测；委托方向承检方提供的一切资料保证是真实、完整、合法、有效；同时确保样品可在国内外合法销售、流通，不用于非法的目的。委托方提供的检测样品用于特殊目的的，应在签订本合同前说明。

The sample submitted by client, others or by mail shall be complete, no-destroyed, safe and fit for test, the information offered by clients shall be true, complete, legal, fit .It shall be sold at home and abroad not used for illegal purpose. If the tested sample will be used for special uses, notification shall be made before signing the contract.

三、委托方对检测报告有特殊要求、对检测结果有测量不确定度要求时需在合同书中说明。

The client shall notify tester by advance notice written in the contract if they have special requirements, requests for measurement uncertainty for test result.

四、本检测委托合同书只接受委托方书面方式的更改。由委托方代表人签字，经承检方重新合同评审后认为可行的，才进行合同更改。合同书更改后即按更改后的合同执行检测。

The contract may only be modified by a variation expressed in writing and signed on behalf of the client, confirmed by tester after the review of the contract.

五、由承检方确定检测方法或对检测方法有偏离时需及时通知委托方，并得到委托方的确认。

FYT shall notify the client in time and make confirm by them if test methods adopted by tester or there is any deviation of the test methods.

六、承检方按委托方的要求发放检测报告。委托方自取检测报告，凭本合同领取检测报告。若本合同遗失，可凭单位介绍信及联系人个人身份证复印件或代理人身份证复印件领取。

Tester will submit test report as the client’s request. The client shall present the duplicate (same) for collection of the test report in our office if they tend to take the test report by themselves. If they missed the contract, they can take the introduction letter of the unit or the photocopy of contract person’s ID or agency person’s ID instead of .

七、委托方对检测报告结果有异议时，应及时向承检方提出。

The client shall notify tester in time if they have some unclear or disagree test results.

八、委托方不得利用检测报告进行非法活动，不得私自更改、涂改检测报告。对上述行为造成的后果承检方不负任何法律责任，并保留追究有关法律责任的权利。

The client shall not make any illegal activity and make any correction and amendment of the test report .Tester will not liable for any responsibility caused by above actions and will reserve the right to the legal responsibility.

九、委托方私自复印的检测报告，未加盖承检方红章的，承检方不予认可。委托方需多份检测报告，应在签订合同前提出，承检方一般不对检测报告复印件加盖红章。

Tester will not confirm those test reports if the client reproduce except in full without prior authorization from FYT and have no red chop signed on it. The client shall notice in advance if they need more than one report .As general, Tester will not sign the red chop on the test report photocopy.

十、承检方作为具有独立法人资格的第三方公正、权威检测机构，其所出具的检测报告完全是基于第三方 正立场，不受任何一方的干预、影响，承检方对检测结果负责，对委托方涉及检测的技术数据、商业数据、知识产权、产品其他性能等信息严格保密。

Tester is the third party、impartial and authoritative test organization with the qualification of independent juridical person, issue test report entirely based on the third party impartial ground and shall not be interfered and affected by any party, make serious confidential for the clients technical data ,commercial data ,property right ,others instruction information concerning the test.

十一、如遇火灾、水灾、爆炸、地震等人力所不可抗拒的情况，而造成样品损坏、遗失的，承检方不对样品的损坏、遗失及检测结果负责。承检方可根据需要取消或延缓执行检测合同。

In no circumstances whatsoever shall tester be liable for any loss or damage of sample、 test report caused by such beyond tester control including but not limited to fire、 flood. 、explosion 、earthquake. Tester may cancel or delay the test contract if necessary.

十二、承检方保管检测样品（检测中消耗的样品除外）至检测程序结束，即检毕后45天。逾期委托方仍未领取，承检方将其做无主样品并自行处理。如委托方在签订本合同书时已做出说明，承检方应根据委托方的要求处理检毕样品。

The sample not destroyed in course of testing may at the sole discretion of FYT deemed abandoned and or destroyed after the expiration of 45 days after the date of test report unless special arrangements have been made in advance with the client in writing in regard to the disposal thereof.

十三、本合同一式两份，承检方、委托方各执一份。
